- Release date is less than three years.
- No current domestic violence related convictions.
- Reside in and work in one of the following areas: Anchorage, Fairbanks North Star Borough, Mat-Su, Girdwood, Kenai, Eagle River, Chugiak, Soldotna, Yakutat, Juneau, Ketchikan, Sitka, Skagway, Haines, Hoonah, or Petersburg.
- Must be able to provide a "clean" urine sample (no prescription narcotics or street drugs).
- NO WEAPONS, ALCOHOL, OR CONTROLLED SUBSTANCES IN THE HOME.
The Electronic Monitoring (EM) program allows inmates who meet certain requirements to serve time at home. Inmates can maintain employment, access community-based treatment, perform community work service, address medical issues, and attend religious functions. There is a weekly cost associated with the program. A fee of either $12.00 or $14.00 a day will be assessed.
DO NOT APPLY if these requirements cannot be met. The current conviction, prior convictions, success on probation and parole, and involvement in treatment related programs will be considered.
- NO CALLING FEATURES such as call waiting, caller ID, voice mail.
- MUST have long-distance carrier. NO long distance charges will be incurred by Electronic Monitoring equipment.
- MUST HAVE ONE CORDED TELEPHONE.
- The telephone line must be clear of all electronic equipment - no fax machines, computers, or answering machines, no dial-up internet connections.
- DSL Internet connections must be removed.
- ALL PAYMENTS ARE TO BE MADE IN CASH, MONEY ORDER, OR CASHIER'S CHECK ONLY!
- All installations are done Monday - Friday within business hours. Be prepared to provide a urine sample and the first week's payment (either $84 or $98).

These forms should be submitted to the appropriate Electronic Monitoring Office responsible for the area where residing, between Monday to Friday from 8:00 A.M. to 4:30 P.M. The EM Officers will process the received packet and a home inspection will be scheduled once appropriate. Once the home has been inspected and the packet is approved, a "Report" date will be given.
EM INFORMATION FORMS ARE REQUIRED FOR PROSPECTIVE ELECTRONIC MONITORING CLIENTS. IT IS RECOMMENDED THAT RESIDENTS AT THE APPROVED RESIDENCE BE AWARE OF THE EM Permission to Enter and Search Form.
- For individuals who were SENTENCED to a 1st time DUI or 1st time Refusal on or after 1/1/2017 and the offense occurred prior to 7/8/2019, please complete and return the following forms with a copy of your judgment:

- For individuals who are in a rural area who were SENTENCED to a 1st time DUI or 1st time Refusal on or after 1/1/2017 and the offense occurred prior to 7/8/2019, the following forms will apply:

- EM Terms and Conditions for First Time DUI / Refusals (Home Confinement)
- For all other offenses, please complete and return the following forms with a copy of your judgment prior to your remand date, this includes 1st DUI and 1st time Refusal when the offense date occurred on or after 7/9/2019:
